The novel begins with Rose Hathaway, a half-human/half-vampire (Dhampir), and her best friend Vasilisa "Lissa" Dragomir, a Moroi princess. They have been on the run for two years from the authorities of the St. Vladimir's Academy where they both went to school. Though the reason is not yet revealed, Rose implies that Lissa was in some kind of danger while at St. Vladimir's, and as her unofficial guardian (a protector against the Strigoi--the immortal and evil vampires keen to destroy the Moroi race and turn them into one of their own kind) orchestrated their escape from the heavily guarded school and whatever was haunting Lissa there. However, they are caught by a group of the schools guardians led by Dimitri Belikov, and are sent back to the Academy in Montana.
Rose and Lissa's lives are complicated because they were in a car accident a few years before which claimed the lives of Lissa's parents and brother, but left the girls unhurt (it is later revealed that Rose died in the accident as well, and Lissa unknowingly healed her). After the accident, the two girls developed a rare magical bond which allows Rose to feel Lissa's emotions and sometimes see things through her. Lissa also finds herself capable of healing things, the result of a rare spirit magic she possesses, which Rose and a previous teacher with the same ability, Ms Karp, urged her to hide. Once back at the Academy, Rose and Lissa adjust back to the school's work and complex social life. Rose finds herself behind on her guardian education, and is put to take extra practices with Dimitri, if she wants to become Lissa's other official Guardian after graduation. While training with Dimitri, Rose finds herself increasingly attracted to him.
Lissa begins to receive threats that reminds her of the danger she felt before leaving school. The frightening threats continue as Rose works hard to keep Lissa's fragile state intact from the danger as well as gossip from classmates like Mia, and ones she perceives as dangerous, such as Christian Ozera, whom Lissa strikes up a secret friendship with. On the night of the school dance, Lissa is kidnapped by a group of guardians under orders from her dying uncle, Prince Victor, who has been planning this since managing to discover that she has healing abilities. He therefore has Lissa abducted from the school, and goes as far as ensnaring Rose and Dimitri with a lust spell, in hopes that they will be too distracted with each other for Rose to find Lissa through their bond. However, his plan is foiled by Dimitri, who manages to break the spell while being with Rose, and they set off to save Lissa. Lissa is forced to heal Victor, nearly killing herself from the strain of the power. She is saved by Rose and the stow-away Christian, and Victor is apprehended. While being held, he manages to get his Academy student daughter Natalie to turn into a Strigoi, which is done by an evil deed--in her case, killing another Moroi. As a powerful Strigoi, Natalie attempts to free her father and almost kills Rose, who is saved by Dimitri, who then kills Natalie. The novel ends with Lissa working to keep her spirit-magic ability under control, and Rose recovered and in love with Dimitri, who insists they cannot have a relationship because they are Lissa's guardians.
